The form of the additional declaration is settled in the appendix of the regulations of Cabinet of Ministers issued on September 19, 2006, No.780 „Regulations on the form of additional declaration of income, monetary and other savings, property and the change in value thereof, and order how the State Revenue Service on the basis of calculations determines the amount of the taxable income”.
